This article deals with the use of leeches in the treatment of herpes zoster, also known as shingles, caused by the varicella zoster virus (VZV). Recent studies suggest that exosomes, which play a crucial role in the development and spread of herpes, can be disrupted by the treatment of leeches. This disruption leads to a reduction in the pain and burning sensation and stops the progression of the conditions. In Ayurveda, herpes zoster is associated with pittaj visarpa, and the principle of treatment includes bloodletting (raktmokshan) and local treatment with medicines (pradeha). The article presents a case study of a 65-year-old woman with type 2 diabetes who was treated for herpes zoster with leech therapy according to Ayurvedic principles. Significant pain relief was noted after the first session of leech therapy, which continued to improve in subsequent sessions. The combination of leech therapy and internal medication brought complete relief. The study concludes that Ayurvedic pain management through leech therapy is an innovative, simple and cost-effective treatment for herpes zoster and points to the need for further research to investigate its practicality alongside modern treatments and its effects on exosomes.
